Recent research from the University of Waterloo indicates that long-term stress levels, as measured through hair samples, may provide critical insights into mental health risks among children suffering from chronic physical illnesses (CPI).

Importance of Hair Cortisol as a Health Indicator

The study underscores the role of elevated hair cortisol, a steroid hormone, as a significant early warning signal. This biomarker could assist in identifying children living with CPI who are at heightened risk for mental health challenges. Such insights could inform prevention and treatment strategies aimed at improving their overall well-being.

Prevalence of Chronic Physical Illness in Children

In Canada, approximately 40 percent of children are affected by chronic physical illnesses, a figure that has been steadily increasing over the years. These children are at a substantially higher risk of developing mental health issues compared to their healthy counterparts, leading to an increased likelihood of poor quality of life, suicidal ideation, and heightened reliance on healthcare services.

Emotional Toll of Chronic Illness

“Living with a chronic illness entails daily challenges that include medication management, school absenteeism, and lifestyle adjustments, all of which can exert a significant emotional toll,” remarked Emma Littler, a PhD candidate in Public Health Sciences at Waterloo and the study’s lead author.

“Our findings indicate that persistently high stress, as indicated by hair sample analysis, can help identify children with CPI who are most susceptible to mental health problems. This insight paves the way for earlier and more targeted support.”

Study Findings

The study tracked 244 Canadian children with chronic physical illnesses over a four-year period, using hair cortisol levels as a biological marker to assess stress over time. The researchers discovered that over two-thirds of these children exhibited consistently high cortisol levels, correlating with increased symptoms of depression, anxiety, and other mental health concerns compared to their peers with decreasing cortisol levels.

The analysis revealed that children with declining cortisol levels exhibited fewer symptoms of anxiety, depression, and behavioral difficulties than their counterparts with persistently high cortisol levels.

Intervention Opportunities

“Early identification of these risk factors can enable healthcare providers and families to intervene before emotional and behavioral issues become entrenched,” stated Dr. Mark Ferro, a professor in Waterloo’s School of Public Health Sciences and co-author of the study.

“Hair cortisol serves as a non-invasive, easily collectible biomarker, which could eventually be employed to screen children and monitor the efficacy of treatment or support programs in reducing stress.”

Further Research on Biomarkers

The study titled “Association between hair cortisol and psychopathology in children with a chronic physical illness” has been published in Stress and Health. Ferro and his colleagues, in collaboration with researchers from McMaster University, have also published findings indicating that biomarkers found in the blood of children with CPI may predict future mental health challenges. Their study, “Inflammatory biomarkers predictive of psychopathology in children with physical illness,” uncovered correlations between certain blood signals and the deterioration of mental health over time, while others were associated with improvements. These findings suggest that routine blood tests, combined with mental health assessments, could aid healthcare providers in identifying children in need of additional support at an earlier stage.
